Notice Description

The Greater Manchester Combined Authority wishes to establish one contract for the delivery of an earlyintervention service to support people in Greater Manchester who are at risk of falling out of the labour market,or who are newly unemployed with a health condition or disability. This will be a three year contract commencingin March 2019 and will aim to support around 14,000 people over the duration of the contract. This programmewill support two groups of people: * In-work: People who are employed but off work due to a health condition or disability, and who are at risk offalling out of the labour market. * Newly unemployed: People who have recently (within the last six months) fallen out of work, and face barriersreturning to work, due to a health condition or disability. The maximum value of this contract is PS6.5 million for the duration of the contract. We estimate this to cover inthe region of 14,000 programme participants over the duration of the contract the core minimum components of the service are expected to be: * Condition management, patient activation / motivation & self-care advice * Rapid access to musculoskeletal & mental health support * Occupational Health Advice * HR & Employment Advice * Careers advice & job brokerage * Support for psycho-social issues, e.g. debt, housing, relationships etc.
