Notice Description

Further to the Invitation to Quote process halted on 20 March 2020, in response to Covid-19 pressures, North East Hampshire & Farnham CCG and Surrey Heath CCG are now republishing this requirement. The participating CCGs are working in collaboration to commission a single Home from Hospital service across both CCG footprints. The overall aim of the Home from Hospital service is to provide a responsive and time-limited service to individuals who are medically fit for discharge from acute hospital or community unit, through providing and signposting onto, advice, information and person centred support for these individuals and their carers to safely resettle at home, and to live healthy and independent lives. The Service provider(s) will link individuals with appropriate interventions (e.g. volunteers and community groups) that can facilitate support and connections for an individual and or their carer to settle back at home and restore "normal" daily functioning. It will allow people to regain their confidence after a period in hospital or after a short stay in the emergency department in a way that will promote their independence and reduce the likelihood of hospital readmission or Long Term Care - there will be prompt access which enables timely discharge. Bids from single providers, legally established consortia and/or partnerships are welcome; partnerships should note they must select a lead partner to become the legal entity on the contract. NB: This is a 2 year contract, with one extension option of 12 months. Additional information: The ITQ process is being managed by NHS South of England Procurement Services (SoEPS) using the In-tend electronic tendering portal. To access the ITQ documents, interested parties will need to register (free of charge) as per the guidance below. Instructions for Registering on the SoEPS Commissioning Support In-tend e-Tendering Portal NB If you are already registered on the portal, login as usual and go to step 8 below. 1. Browse to the SoEPS Commissioning Support portal - https://in-tendhost.co.uk/soepscommissioning 2. Click the "Register" link on the left-hand side of the page. 3. Enter your correct business and user details. 4. Note the email address chosen and Click "Register My Company" when complete. 5. You will receive an email with your unique password (keep this secure) 6. Login to the portal with the username/password. 7. You will be prompted to change your password on first login. 8. Hover your mouse over the "Tenders" button on the main toolbar and choose the following option: 'Current' which shows a list of all the advertised opportunities on the system (i.e. open to all) 9. Scroll through the opportunities and select 'F13394_ITQ2'. 10. Click on the 'View Details' button and 'Express an Interest' where required to do so to gain access to the quotation documents. 11. Click the 'Opt-In' at the bottom of the page to start building your ITQ submission. This will move the project over to your 'My Tenders' page. (This is a secure area reserved for your projects only). Further guidance can be found in the 'Help' tab accessible at the top of the screen.
