The ICS requires that solutions provided can immediately be upscaled in alignment with trajectories the Integrated Care System submitted and agreed with NHSE. The partnership of providers in Surrey requires, from the outset, interoperability with in situ primary, community and secondary care electronic patient record systems, a future proof solution which can support both spot and continuous monitoring and invite suppliers to articulate a service offering for the issue and retrieval, maintenance, and infection control of remote monitoring devices, once the service goes live. The initial ambition is to support @Home monitoring and Virtual Wards e.g., acute frailty and acute respiratory infection pathways, maturing rapidly to provision a solution which supports people along the care continuum, including providing higher levels of monitoring according to needs. The ICS seeks to demonstrably achieve both admission avoidance and early discharge from acute settings. Surrey Heartlands have a strong organisational desire to create the highest standard of end-to-end patient experience and would request suppliers who tender provide solution designs and narrative focused on articulating this goal. Other service providers across Surrey will seek to utilise this platform: Cancer Services, Adult Social Care and Children/Young People and Maternity. There will be potential opportunities for suppliers to create further Virtual Care pathways to support additional services on the platform, once implemented and the operational need for further devices beyond the initial capacity. Cancer Service have immediate investment opportunities in financial year 2023/24.
