Care will include complex symptom control, specialist palliative rehabilitation and coordination of care at the end of life. Palliative care education and advice is provided to other health care professionals. All patients admitted to the specialist community care services are assessed by the medical team/nursing team/ multidisciplinary team. The MDT devise a care plan / goals for the patients care which is agreed by the patient / family. An agreed set of services is put in place. The care given and planned is reviewed on a regular basis by the MDT or appropriate health care professionals through formal review criteria. For patients who require discharge from the service there is formal discharge criteria and agreed pathways for referral to services. The Contract will be awarded for a 5 year period on a 3 plus 2 year extension option",
