The programme will: Undertake a comprehensive review of current Care at Home services and practise and provide evidence based, context-specific and tailored recommendations for service improvement and transformation; Develop the service transformation plan and implement it over a 2 year period; Delivery of a communications and change management strategy; Collaborate with stakeholders to co produce the plan and implement it in line with the partnership ethos of the HSCP.
